TELEMEDICINE

In rural Alabama, accessing expert HIV medical care is complicated by poverty, stigma, and a lack of transportation. With telemedicine, Five Horizons- Montgomery provides access to medical care in our rural clinic through video teleconference – drastically reducing the burden of travel and related expenses. Telemedicine is a method of conducting a traditional face-to-face doctor visit via real-time video conference. With high-speed internet, high-definition video, and state-of-the-art Bluetooth medical equipment, specialists can treat patients virtually anywhere without the need for travel. Telemedicine provides a cost-effective means of transcending the barriers to care faced by rural Alabamians living with HIV/AIDS.

Psychotherapy Session

PEER COUNSELING

The peer mentors provide education and outreach services to infected persons offering referrals and linkages to medical/dental care, consumer advocacy groups, inpatient/outpatient substance abuse treatment, emergency and transitional housing, case management, secondary prevention counseling and food distribution programs.
The peer mentors work very closely with their local medical clinics, AIDS service organizations and community-based organizations that provide HIV-specific services to infected persons and high-risk negative individuals.

HOPWA(EMERGENCY HOUSING)

The Housing Opportunities for Persons With AIDS (HOPWA) Program is the only Federal program dedicated to the housing needs of people living with HIV/AIDS. HOPWA provides short or long-term housing assistance to HIV-positive, low-income persons and their families. The purpose of the program is to enable clients to maintain decent, safe, and affordable housing and to prevent homelessness.
